Almost every homeschooling family builds a record system in September and abandons it by half term. The system is rarely the problem in principle. It is that it was designed on a good day, for a version of the week that does not happen often.
The version that survives is the one that still gets done on the bad Tuesday, and that means it has to take well under a minute.
Three things, every time
The date. The subject and roughly which part of it, where Unit 3, Lesson 12 is plenty. And one word about how it went: easy, about right, or difficult.
That third field is the one people leave out and the one that turns out to be most useful in March, because it tells you where to look when something has not stuck.
What kills a system
Every one of these looks reasonable in September. Tick anything your current system asks of you.

Plan four days and record five
A family that plans five days and manages four has failed at something every single week. A family that plans four and manages four has not. The work done is identical.
Recording is the same. If the habit assumes a perfect week, every ordinary week produces a gap, and gaps are what make people stop.
The fourth thing, occasionally
Once in a while something happens that no log captures. She finally understood fractions. He reads better lying on the floor. A bad two weeks turned out to be a cold rather than a problem.
Write those down the day they happen, in a sentence. In three years they are the only part of this you would not want to lose, and by next month you will have forgotten every one of them.
If it already collapsed this year
That is the normal case rather than the exception, and more is recoverable than it feels like. The method is in when you have kept nothing since October.
